Standard Forms 

These forms are required by every student in the district to have on file.  These forms are required when you first register with the district and when your student is registered for middle school and high-school.  Notice that the elementary and the secondary forms are slightly different.

Elementary(gr K-5)

Secondary(gr 6-12)

Medication 

If your student may require medication to be taken at school, please refer to the following guidelines Medication at School

Print out the Medication Consent Form and return it to your school nurse, if your student will require medications to be given at school. 

Asthma

If your student has been diagnosed with asthma, please print out the Asthma Questionnaire and return it to your school nurse. If your student needs a more detailed plan for school, please have your doctor fill out the Student Asthma Action Card and return it to your school nurse. 

The Meridian School District has an Emergency Administration of Medication policy which allows students with special health needs to carry and/or self-administer their own inhaler.  If you prefer your student carry his/her own inhaler while at school please speak with your school nurse and return the following forms to her based on their grade level in school.

Elementary(gr K-5)

 Written authorization from physician

 Written authorization from parent

Secondary(gr 6-12)

Written authorization from parent

Bee/Wasp Allergy

If your student has a bee, wasp or insect allergy, please fill out the Bee/Wasp Allergy form and return it to your school nurse.

If your student may need specific medical intervention for their allergy, please have your physician fill out the Allergy Management Plan and return it to your school nurse.
