Diabetes
If your student has diabetes, please fill out the Student with Diabetes Information Sheet and return it to your school nurse.

Food Allergy
If your student has an allergy to food of any kind, please fill out the Food Allergy Assessment form and return it to your school nurse.
If your student may need specific medical intervention for their food allergy, please have your physician fill out the Allergy Management Plan and return it to your school nurse.
If your student requires an Epipen, please send one to school along with a Medication Consent Form.
This letter may be sent home to an entire class if there is a student in that class with a food allergy; Food Allergy Class Letter.

Head Injury
The Meridian School District likes to identify students who have suffered a brain/head injury at some point in their life. Research has shows that academic difficulties often do not show up immediately with some brain injuries. By identifying and gathering information on these students we will be better able to assist them if necessary. If your student has had a head injury, please fill out the Head/Brain Injury form and return it to your school nurse.
