Immunizations
All Idaho schools are required to implement Idaho Code, 39-4801 and Title 2, Chapter 15 "Immunization Requirements for Idaho School Children." This code requires all students enrolled and enrolling in Idaho Schools must show verifiable documentation of immunization prior to school entrance. This documentation must show date and type of each dose administered. This must be stamped by a physician or health department. Parental recall of dates and types of immunizations received are not acceptable.
The minimum completed immunizations are 5 DTP/DTaP/DT/Td, 3 Polio, 2MMR(Measles, Mumps, Rubella), and 3 Hepatitis B. If there are religious, medical or personal reasons for not completing immunizations an exemption form must be signed. Your school nurse will be able to provide this form for you.

Other recommendations for vaccinations include:
Hepatitis B: Series of 3 shots given over 6 months. It is recommended that everyone in Idaho have this series completed. For anyone born on or after November 22, 1991, the series is required for school entrance.
Influenza: Is now recommended for all children and adults starting at 6 months of age.
Tdap(Tetanus, diptheria, accellular pertussis): Recommended for all 11-12 year olds and for anyone over 11 who are in contact with infants less than 12 months of age.
HPV(Human papilloma virus): Series of 3 shots, recommended for girls age 11-12 years old and all older girls/women through age 26 who have not been vaccinated. This one is a new vaccination that helps prevent cervical cancer.
Varicella/Chicken Pox: A second dose is now recommended to all children and adolescents who have only received one dose.
Hepatitis A: Is a 2 shot series, recommended for anyone in Idaho, 2 years old and older who have not had the series.
Meningococcal conjugate: Recommended for adolescents age 11-18 years old, and all college freshman living in dormatories who have not been vaccinated.
